You can access portions of your electronic medical record online with the MyPHC Patient Portal powered by FollowMyHealth. That includes things like your medical history, most test results, and immunization records.

How to Request Medical Records

The fastest and easiest way to access your Medical Records is online with the MyPHC Portal. Patients may access their online medical records free of charge.
If you are not a portal member, please fill out the form in the green box to the left and click submit to request a MyPHC Patient Portal powered by FollowMyHealth account.

Unable to get what you need from the portal? Printed copies are available upon your request.
To request a copy of your Medical Records, complete the Medical Records Release Form below.

*The charge for this service is in accordance with North Carolina Law and The HITECH Act.

Have a question about your Medical Records request? You may leave a message with our Medical Records Department by calling 704-978-3546.  A representative will review your request and return your call the same day.
We are unable to accept Medical Records requests by phone. All Medical Records requests must be received in writing.
