The medical team at PHC IM & WM is dedicated to provide comprehensive primary care to adults, age 16 and older, in a professional and compassionate manner. As primary care specialists, we treat the individual and their healthcare needs by creating a personalized plan of care as unique as the patient. We specialize in the diagnosis and treatment of acute illness and long-term management of chronic conditions. We have a passion to collaborate with the patient in the prevention of diseases and to help optimize wellness.
As “internists” we treat older teens and adults of all ages. Our goal is to treat the whole patient including wellness, disease diagnosis, prevention and treatment, acute care and much more.
As internal medicine providers, we treat the “whole” patient. Serving patients from older teens to adults of any age, we specialize in the diagnosis, treatment and prevention of diseases, in-office procedures, and wellness services.

Dr. Manish Patel is Board Certified in Internal Medicine and Obesity Medicine. He has gone through extensive training with the American Society of Bariatric Physicians, now called Obesity Medicine Association. He started Piedmont Healthcare Weight Management 6 years ago. Jennifer Streu has joined his team. She is a highly motivated nutritionist, fitness instructor and lifestyle coach with a passion for nutritional education.

Piedmont Healthcare Weight Management’s mission is to provide a personalized, comprehensive program that leads to improvement in your overall health. We are not here only to help you look better. Feeling better, reducing or eliminating medications and living life more fully are our ultimate goals for you.
Our program incorporates four principal tools: nutrition therapy, pharmacologic (drug) therapy, physical activity and behavioral therapy. We believe in educating our patients as we work with them to achieve the best physical state possible.

It is our firm belief that nutrition is the cornerstone of any healthy lifestyle or weight loss endeavor. Pills can be helpful tools but food is the most important medication! We will review our medical nutrition plan with you one-on-one and answer any questions you may have and provide supportive written materials.
Many of our patients are on multiple medications to treat obesity related medical issues. Some of these medications could cause weight gain and we will work with your physician to replace, reduce or eliminate those medications that can hinder weight loss.
We will also discuss how exercise and non-exercise activity affect your weight loss and healthy lifestyle goals. We can help you set exercise and fitness goals if needed.
We like to get to know you well and provide ample time for your questions. Please allow an hour to and hour and a half for your first visit. We do our best to accommodate little ones but be prepared with snacks or activities if you must bring small children. An hour and a half will seem like an eternity to them!

Follow up visits are usually monthly but the schedule will be individualized based upon your needs.
Follow ups will include body composition analysis, ongoing review of the nutrition plan, evaluation of your response to medications and review of any labs results. Our team strives to provide ongoing positive feedback, motivation and information.
Regular follow up visits are key for long-term weight loss success. If you are struggling or feel you are not losing, that is when you need us the most – just as you would need to see your cardiologist if you were having issues with chest pain.
Keeping weight off is often the most difficult for folks. We encourage scheduled follow up visits to provide accountability, ongoing support and evaluation. Your maintenance lifestyle will be unique to you. Our team will help you determine which foods work for you for long term control.
If you have an obesity related medical condition such as high blood pressure, diabetes, high cholesterol, thyroid disorder, or sleep apnea, your insurance company may cover your office visit. If you do no have insurance, please call our office for approximate costs.
Disease Prevention- Chronic diseases are the leading cause of death worldwide. According to the CDC, 75% of all deaths in the United States are attributed to just 10 causes- 8 of which are chronic diseases such as Heart Disease, Cancer, and Diabetes. These afflictions are linked by common and preventable factors like obesity & high cholesterol and are directly influenced by diet, physical activity, sleep, and tobacco use.
Blood Sugar Stabilization (for Insulin Resistance, Pre-Diabetes, Diabetes)– Poorly managed blood sugar, aside from increasing the risk for developing Diabetes, commonly leads to weight gain, fatigue, and cravings. Autoimmune Protocol (AIP)- A food-based approach designed to put autoimmune disease into remission or at the very least, slow the progression. AIP is a temporary elimination diet that focuses on the removal of certain foods that create inflammation and overstimulation of the immune system. Once there is a noticeable improvement in symptoms (usually after 30 days), foods are reintroduced back systematically to assess whether or not they are triggering an inflammatory response.

These foods are high in anti-nutrients which can interfere with the absorption of important vitamins and minerals. We can help optimize your vegetarian diet to ensure all vital nutrients are consumed.
Fitness Programming- Creating appropriate individualized Fitness plans is both an Art and a Science. Unfortunately, most people are more confused and misguided than ever when it comes to fitness and subsequently end up engaging in physical activity that over-stimulates certain muscular groups, leading to injury. A strong and safe individualized plan will take the following into account: baseline fitness level, existing muscular and structural imbalances, motivation level, time constraints, and numerous other lifestyle challenges.
Sports Nutrition- Your body requires a specific balance of nutrients from carbohydrates, protein, and fat to fuel any type of prolonged physical activity. Failing to refuel your body properly will eventually lead to poor performance and results. This applies to athletes who engage in high-level sports as well as to those who perform high quality exercise on a regular basis.
Family Nutrition- Recent research suggests the benefits of a home cooked meal may be outweighed by the stress and pressure involved in actually preparing it. Additional challenges include budget & time constraints, different food preferences, and a lack of general nutritional knowledge. The more insidious complications involve conflicting nutritional needs among family members based on age, activity level, and health issues. Despite these issues, there are simple ways to make sure everyone in your family receives a delicious, balanced meal that meets their individual needs.
Our Nutritional services are not billed through insurance. However, any necessary labs or office visits with a Medical Provider qualify for insurance billing. The cost of ALCAT test is dependent on number of foods and chemicals tested.
I just want to personal thank each of you for seeing me that one day in February this year. When I first came in to see both of you I was 277lbs, wearing a size 44 dress slacks, snoring every night, low energy, poor life style choices, bad health numbers, borderline on some potential serious long term health issues and I could go on.
Both of you gave me the understanding of how my body works and what I was doing to it and where I was headed. Both of you also gave me the tools to better myself, (THE GREEN BOOK!!) Truly a life style change! Knowing that it took me a few years to get to where I was made me realize that journey back to good health would be a marathon in itself.
Fast forward to September 23rd, 2015. I have lost nearly 60lbs, I have a new life style, crazy off the charts energy, love to exercise, selfish about what goes in this body, consider food as a fuel now, love that I now fit into size 38 dress slacks, have added a few more hopefully A LOT of years to my life and most importantly will be here for a long time to come for the three most precious and important people in my life, my Volin women!
Speaking of marathon… I am running my first half marathon on OCT. 3rd here in Mooresville and YES I will send another picture of this goal that I intend to smash.
