Dr. Bruce Seaton offers immigration medical examinations (Form I-693) in the Lake Norman area, providing a clear, efficient experience for individuals and families.

Your Immigration Examination Appointment

What Do I Need to Bring?
1. You will need the I-693 form, printed out with Part 1 and 2 (and Part 3 if interpreter is needed) completed in black ink only. There are 14 pages in this form. At the top of each page there is a space for you to write in your full name (as it appears on your identification) and Alien ID number (if you have one). Please be sure that all these spaces are complete, match, and are done in black ink only.
**Note that the space for your name on the last page is further down on the page (about 1 ½ inches from top of page).
Be sure the expiration date of 09/30/2027 appears in the upper right-hand corner of the first page. This form can be found at
uscis.gov/i-693 , the link is half-way down on the first page of the website.
2. You will also need a current, non-expired government issued picture ID. (ie: Driver’s license, State ID card, Passport)
3. Lastly, you will need your vaccination record. This must be an official vaccination record from the provider’s office or organization where they were received.
**If your vaccination record is in another language other than English, we would appreciate a translated version along with the original if possible.

What is the Cost of the Exam?
1. The total cost of the exam is $475.00*. This charge includes the exam and the required lab work.
2. If your tuberculosis test comes back positive, then we will need to perform a Chest x-ray (located at our site) for an additional $85*.
3. Vaccinations are additional charge, and depend on age and history.

How Long does the Appointment Take?
1. The exam is completed in one visit, including the lab work and any vaccinations that may be required. On average, expect to spend 30-45 minutes at the office; some appointments do take longer, therefore we ask that you allow plenty of time to complete the appointment.
**You MUST arrive 30 minutes before your appointment time!
2. As mentioned above, if there is a positive TB test, we will call you to come back to the office and have a Chest X-ray.

How Long does it take for the Packet to be Completed?
1. It will take 10-14 business days for your packet to be completed. You will be called when the packet is completed and ready for you to pick it up. You will be provided with a copy of the completed packet for your records.
2. The original is in a sealed packet for your immigration case worker.
**NO ONE CAN OPEN THIS SEALED ENVELOPE EXCEPT FOR THE IMMIGRATION CASE WORKERS**

What if I Need Vaccinations?
1. We can perform your vaccinations here, at the office, during your exam. Vaccination requirements are based on your age and what vaccinations you already have had.
2. There is an additional charge for vaccinations, depending on what is needed.

- Generally, adults need to have Tdap, MMR, Polio, Hepatitis B, Varicella (or history of having chicken pox) and a flu shot for this season (September 1 to April 1).
- Patient under 18 may require additional vaccinations (Hepatitis A, Meningitis), and patients over 65 may not require some of these vaccinations but may require a pneumonia vaccination.

No, fasting is not required. You can eat and drink normally before your appointment unless instructed otherwise.
What if I don’t have my vaccination records?
