Sherlock Internal Medicine Works Toward Patient-Centered Medical Home Recognition

According to the NCQA, a Primary Care Medical Home (PCMH) practice inspires quality in care, cultivates more engaging patient relationships, and captures savings through expanded access and delivery options that align patient preferences with payer and provider capabilities. As a part of the ongoing commitment to the highest level of care, the providers of Sherlock Internal Medicine has implemented many quality improvement projects since September 2015 working toward PCMH recognition.

The practice worked to improve in the following areas:

  • Decrease in number of patients who need Zostavax immunizations – improved scores by 14%.
  • Decrease in number of patients who need Prevnar immunizations – improved scores by 4%.
  • Decrease in the number of patients who are overdue for their Annual Wellness Visit – improved scores by 16%
  • Decreased the number of less than 30 day hospital readmissions by 4%
  • Implemented a campaign to women who are overdue for their mammogram- improving scores by 6% from Quarter 3 2015 to Quarter 1 2016
  • Reached 95% compliance with completing medication reconciliations at each patient visit.
  • Focused outreach to patients who have uncontrolled blood pressure
  • Focused outreach to patients who have an A1C greater than 9
  • Focused on cost reduction, by making generic medications available to patients.
  • Completed renovation of office to provide a more comfortable and inviting atmosphere for patients
  • Implemented a Care Coordination department staffed with nurses and a clinical team. Their role is to reach out to patients by phone in-between their visits to the office, working with patients to help them meet their health goals (lowering blood pressure, lowering blood sugar, increasing exercise, weight loss, etc.).
  • Conducted patient satisfaction surveys to learn best what patients would like to see improved within the practice
