If you are not a current PHC patient, medical record requests can be submitted to our Provider 1st team by fax or email. For any questions about medical records, please call the support line for assistance.

For Current Patients
Accessing Online Medical Records
Piedmont HealthCare Patient Portal
If you are a current patient, you can access portions of your electronic medical record online with the MyPHC Patient Portal powered by FollowMyHealth. That includes things like your medical history, most test results, and immunization records.
Already a Portal Member?
Need a Portal Account?
- Go to FollowMyHealth.com and create an account
- Once your account is created and you are logged into FMH, go to ‘My Account’ on the top right menu, click ‘Connections’, and click ‘Add Organization’.
- Search by zip code 28625. Choose Piedmont HealthCare.
- It will then tell you your request is pending with Piedmont HealthCare, as well as other information about how you will be notified when your request is approved, and that you may be contacted for further proof of identity. Click Continue.

To request a copy of your Medical Records, complete the Medical Records Release Form below.

Have a question about your Medical Records request? You may leave a message with our Medical Records Department by calling 704-978-3546. A representative will review your request and return your call the same day.
We are unable to accept Medical Records requests by phone. All Medical Records requests must be received in writing.
