We do accept most insurances, and we file claims as a courtesy to our patients. Payments for services not covered by your insurance, copayments, coinsurance and/or deductibles are expected at time of service. Please contact your insurance company or our billing office at 704-662-8336 for questions regarding billing. We accept cash, checks, Visa/Mastercard, AMEX and Discover.
For your first visit to our office, we ask that you arrive 30 minutes prior to your scheduled appointment time to complete registration forms or you can download and complete forms on our website.
Please bring your current insurance card(s) and a list of medications/over-the-counter meds you are taking.
Our office has a strict policy on cancellations. If you must cancel your appointment for any reason, please notify us 24 hours in advance. Failure to do so will result in a charge of $25.00 to cover the administrative costs associated with a missed appointment.
We are happy to take your prescription refill requests by phone through our clinical voice mail system. Please allow 48 hours to complete your request. Due to the amount of patients we see on a daily basis, we are unable to honor any refill requests in less than 48 hours.
When leaving a refill request, please be sure to leave your name, your phone number, name of prescription, dosage, pharmacy name and phone number. Be sure to speak slowly and clearly. Without all of the above information, your request could be delayed while we obtain the information necessary to complete it.
By law, the physician must have an up-to-date medical record for all patients receiving prescription medications. This time period can change based on the type of medication you are requesting. If you have not been seen within these timeframes, you will be asked to make an appointment with the physician before the refill can be approved. We apologize for the inconvenience, but this is mandatory and cannot be overridden, despite the circumstances. Please understand this policy also helps us protect our patients and provide the highest level of care possible.

PHC – Advanced Foot & Ankle Center patients use our PhreesiaPad when
they arrive and complete the electronic check-in interview from the waiting room. Our digital solution collects additional patient information, signatures for consent forms and insurance information. Patients can even make their co-payment or pay their outstanding bill.
